What you'll be doing

As a Case Manager you will provide comprehensive support by developing and overseeing personalised care plans, coordinating services, liaising with healthcare providers, monitoring progress, and making adjustments as needed. You will ensure that all aspects of care are aligned with your preferences and goals.

No two days are the same - you'll be:

• Meeting clients and getting to know what matters to them

• Creating care plans that help them stay independent

• Coordinating services and checking everything is running smoothly

• Being the go-to person for clients, families, and care teams

• Helping people navigate their care with confidence

What we're looking for

You might be an experienced Case Manager, or someone ready to step up - either way, you:

• Care about people and love building relationships

• Are organised and good at juggling different tasks

• Communicate clearly and confidently

• Have experience in community, aged care, or similar

• Have a driver's licence and are happy to travel locally

• Have experience with Home Care Packages or aged care programs

• Confidence managing budgets or care plans
